XTRASORB DRESSINGS-AN INEXPENSIVE SOLUTION FOR WOUNDS THAT PRODUCE A LOT OF FLUID Until the advent of Xtrasorb, the dressing of wounds with heavy exudate has always been an expensive, time-consuming process. Many wound professionals have preferred foam dressings which will hopefully last 1 day, or a combination of ABD abdominal pad dressings, gauze pads and wraps which probably need dressing changes at least 3x per day. There is now a new product, Xtrasorb from Derma Sciences, which absorbs and holds an incredible amount of wound fluid. It comes in 2 sizes: 4"x5" holding 80-100cc of fluid, and 6"x9" holding 160-200cc of fluid. Unlike foam dressings, Xtrasorb keeps wound fluid trapped inside its cells, which keeps proteinaceous substances (MMPs) in the wound bed at a minimum. Foam dressings release and recirculate MMPs back into the wound itself, to the detriment of healing. Another real benefit of Xtrasorb is that it does not macerate the periwound tissue because wound fluid is held in the dressing. A word of caution: Xtrasorb has to be used as is, with no cutting to size and shape. And for the record, the 4"x5" size, box of 10, costs $41.10, and the 6"x9", box of 10, costs $78.38. RESCUE OF DIABETIC TOE FROM AMPUTATION--A "TRAUMATIC AND DRAMATIC" TALE Wound care professionals love to hear "wound stories" about others' experiences with various wounds and wound treatments. We also have a success story that we'd like to share with you. A woman, referred to us by a nurse colleague in our community, visited our store. Susan (a pseudonym) told us that she was a type I diabetic, and had undergone kidney transplantation a year earlier. Although she is no longer insulin dependent, she explained that her body still behaved somewhat like a diabetic's, and would for a few years after the transplantation. While clipping her toenails, she accidentally clipped some skin on her big toe. Much to her shock, this small wound became larger, and quickly infected. After visits to several doctors, and trying all the antibiotic and wound treatments prescribed, the wound was still there. Her own doctor informed her, finally, that the only way to resolve the problem was to amputate the toe. Susan was not ready to face this trauma, and luckily was referred to us. We had just finished supplying wound products to another diabetic patient, male, who had injured his big toe while gardening, and had successfully closed up his wound. As described to us, each patient's big toe had a large, black, non-healing necrotic tip. We suggested a simple way to clean up these wounds, add hydration and kill any bacteria that were present. Each patient began to use Anasept Antimicrobial Skin & Wound Gel, applied to a 2x2 sterile gauze, and held in place with a gauze wrap. The application was repeated daily. Both patients reported that the black tip gradually began to shrink until gone, leaving behind a clean toe tip wound that eventually closed. ANTIMICROBIAL PRODUCTS WITHOUT ANTIBIOTICS OR SILVER Silver is everywhere these days, in wound dressings made of foam, hydrocolloid, alginate, hydrogel, et al. The silver in these many products comes from a variety of compounds, so just because a dressing has silver in it does not always guarantee the same antimicrobial results. Silver has some bad characteristics to think about, too: stinging, staining, somewhat slow to kill bacteria, costly. Likewise, there are many, many antibiotic compounds for elimination of bacteria and infection. These are all prescription items, some can be very expensive, and can also adversely affect the healing of the wound. Bacteria are known to build up resistance to some of these prescription items, so the antimicrobial effect is diminished or eliminated.
